Applying for your Dundee National Entitlement Card

Call 433558 to request your Dundee National Entitlement Card form. Once you’ve received it, follow these 5 easy steps to completing and returning your Form.

  1. Details:

    Check your details on the form. Make any necessary amendments in the space provided.

    If any amendments are made to your personal details you must bring the form and the relevant proof of these changes with you to your nearest Council Library or Customer Services at Tayside House.

    For example, if you have moved house you can bring along one of the following as proof of your new address:

    Council Tax Bill, Utility Bill e.g. Gas, Electricity, Telephone (within last 3 months), Bank Statement (within last 3 months), Photo Driving Licence.

  2. Photo:

    If we currently have a recent photograph there is no need to supply another one.

    If we do not have your photograph (or a recent one) you will be told on your form to attach a current passport sized photograph to the form. Passport sized photograph means a clear head and shoulders only image with no hats, sunglasses etc. See the passport website for more details:

  3. Consent to share data:

    Dundee City Council will only use the information you provide to produce, administer, and if necessary reproduce your card. We may wish to share your personal details with departments and agencies of the Council, other Scottish Councils, the Scottish Government and its agencies, and the Registrar General.

    We need your permission to share your details to do this. Please note, sharing information with these bodies will not lead to you receiving junk mail. We can only process your application for any other services that are currently available (or may become available in the future) if you agree to share your data. If you do not agree to share your data, you must apply for these services in full separately, where you will be asked to present proof of your personal data again for each service.

  4. Sign and Date:

    Sign and date the form. The date should be the date that you complete the form.

  5. Return:

    For Application forms for ‘Scotland-wide Free Bus Travel for Older People (Age 60 without companion)’ – you can hand in your form to Customer Services, floor 2 Tayside House or your nearest Council library in person.

    For Application forms for ‘Scotland-wide Free Bus Travel for Disabled People (including companion)’ – you should hand in your form to Customer Services, floor 2 Tayside House in person.

    Please remember, if you've made amendments to your personal details you must bring the form and proof of any changes with you.
