What is Home Care Enablement?
- Enablement will help you to re-learn skills you may have lost, or to develop new skills, in order that you can be more independent and improve your quality of life.
- As well as receiving support from enablement workers, a range of aids and equipment may also be identified which can help you to be independent.
What is a Home Care Enablement Team?
Home Care Enablement Teams are based in the community across Dundee City.
The Enablement Teams are made up of:
- Home Care Enablement Organisers
- Trained Home Care Enablement Workers
- Occupational Therapists/Occupational Therapy Assistants
What do the Enablement Teams do?
The teams provide care and support to enable you to:
- Get home after a stay in hospital
- Remain at home, and live as independently as possible
How do I get the support of an Enablement Team?
- You must be 16 years old or over
- You must be assessed by someone from Dundee City Council Social Work Department, or by a health professional. If you are assessed as needing Home Care support, you will then be supported for a short period of time in the first instance, by the Enablement Team.
What happens after my assessment?
- An Occupational Therapist or Enablement Organiser will visit you and explain the work of the Enablement Team. They will work with you to identify your abilities, and what it is that you would like to achieve. From this, an Enablement Plan will then be written with you, and this will be used to monitor your progress.
- Enablement Workers will then work with you to help you to regain skills which you may have lost, or to gain new skills, in order to support your independence. The approach will be to 'do with you', rather than 'do for you'.
The type of tasks they will assist you with may include:
- Getting in and out of bed
- Washing, dressing and bathing
- Toilet and continence care
How long will I be part of the Enablement Team?
- You can be part of the team for up to a six week period. However, you may be in the team for a much shorter period of time. This will be dependent upon how quickly you regain your independence
- The team will monitor the level of support you require throughout your time with the team. The amount and type of support you require may be reduced as you become more independent.
Will I be charged for services whilst part of the Enablement Team?
- The support that you receive from the Enablement Team will be free of charge for the first four week period.
- Following this four week period, some of the support you receive may be chargeable. This will be explained to you by a member of the Social Work Department.
What happens after Home Care Enablement involvement?
- Some people will be fully independent following a period of enablement, and will not require any ongoing support from Home Care Services.
- Other people may require ongoing support, and you will be involved in the decision about what support you need. It may involve support from Dundee City Council's Home Care Services, or from an approved provider of services.
- Some of the support you require may be chargeable.
How do I let you know how I feel about the Enablement Service?
It is important that we get your views about the service. We appreciate being told when thing went well, but we also need to know if you are unhappy about the service, in order to make improvements.
At the end of your enablement period we will ask you to complete a short evaluation form in order for us to gain your views.
