What to do

Fill in the application form on the leaflet, which can be obtained from, and is to be returned to, any of the addresses above. The leaflet can also be downloaded (Acrobat Reader required) from the 'home page' of this site. Along with the completed form you will need the items listed below:

Proof that you are eligible. This could be a recent telephone or gas bill showing your address.
Current proof that you are receiving benefits.
Your Concession will be added to your National Entitlement Card or you will be issued with a Concession card to use immediately.
