How to get your Leisure Concession Card

Applications can be made in person at your nearest sports centre.

What to do
Fill in the application form on the leaflet, which can be obtained from, and is to be returned to, any of the addresses above. The leaflet can also be downloaded (Acrobat Reader required) from the 'home page' of this site. Along with the completed form you will need the items listed below:

1. Proof that you are eligible.
This could be a recent telephone or gas bill showing your address.

2. Proof of receiving benefits.
This could be a benefit book etc.

3. 1 passport-sized photograph.

A card will be posted out to you within seven working days.
No concessions will be granted until your application is processed.
No concessions will be granted unless you produce your card.
